Cocamidopropyl Betaine Cross-Reactors: What Else to Avoid
If you're allergic to cocamidopropyl betaine (CAPB), "no cocamidopropyl betaine" on the label is not enough. The thing most people actually react to isn't CAPB itself — it's the manufacturing impurities left in it, amidoamine and DMAPA. That means a handful of chemically related surfactants can trigger the same reaction even when CAPB is absent. The short answer
- The sensitizer in a CAPB allergy is usually the impurity (amidoamine / DMAPA), not the CAPB molecule — so chemically related ingredients can cross-react.
- Skip the whole amidopropyl-betaine family and its relatives: cocamidopropyl hydroxysultaine, coco-betaine, cocamidopropylamine oxide, lauramidopropyl / oleamidopropyl betaine, and cocamide DEA / MEA.
- "CAPB-free" is not the same as "safe for you": if you're patch-test positive, you're statistically more likely to react to other surfactants too — including alkyl glucosides, which were named Allergen of the Year in 2017. Patch-test, don't assume.
Why "no cocamidopropyl betaine" isn't enough
Cocamidopropyl betaine is a coconut-derived amphoteric surfactant used to boost foam and thicken shampoos, body washes and baby washes. But the research on why people react to it points at the ingredients it's made from rather than the finished molecule: contact dermatitis to CAPB is generally attributed to residual amines left over from manufacturing — cocamidopropyl dimethylamine (amidoamine) and 3-dimethylaminopropylamine (DMAPA). Because those impurities, or the same amidopropyl backbone, can turn up in related surfactants, a product can be free of the exact words "cocamidopropyl betaine" and still carry something that provokes the identical reaction. So a reliable avoidance list has to cover the family, not just the one name.

The honest caveat: "CAPB-free" doesn't mean "safe for you"
Here is the part most "CAPB-free" marketing leaves out, and it matters. Avoiding the cross-reactors above removes the most likely triggers, but it does not guarantee your skin will be happy. Many CAPB-free shampoos rebuild on alkyl glucosides — decyl glucoside, lauryl glucoside, coco-glucoside — and alkyl glucosides were themselves named Allergen of the Year in 2017, with cross-reactivity between the different glucosides being common. On top of that, anyone with a confirmed surfactant allergy has a higher background chance of reacting to more than one surfactant. So "CAPB-free" and even "glucoside-based" are starting points, not guarantees. The only claim that's actually about your skin is the one a patch test gives you.

- Glucosides — decyl glucoside, lauryl glucoside, coco-glucoside (mild, but see the caveat above).
- Amino-acid cleansers — sodium cocoyl glutamate, sodium cocoyl glycinate.
- Isethionates — sodium cocoyl isethionate.
- Amphoacetates — disodium cocoamphodiacetate.
- None of these is "hypoallergenic" for everyone — they're the options most often tolerated by people avoiding CAPB, which is not the same thing. Check them against your own patch-test results.
It's not only shampoo
If your reaction hasn't fully cleared after switching shampoo, the allergen is usually hiding elsewhere. Cocamidopropyl betaine and its relatives are common in body wash, hand soap, facial cleanser, baby wash, toothpaste and even cleaning wipes — and CAPB reactions often show up on the eyelids, ears and neck rather than the scalp. Audit those at the same time, and treat any "free-from" claim on them with the same label-reading you'd give a shampoo.

Is coco-betaine the same as cocamidopropyl betaine?

"Coco-betaine" and "cocamidopropyl betaine" are amphoteric coconut-derived betaine surfactants from the same family; a label may use either name, and both carry the residual-amine risk. Is decyl glucoside an allergen too?
It can be. Alkyl glucosides — decyl glucoside, coco-glucoside and their relatives — were named Allergen of the Year in 2017, and cross-reactivity between the different glucosides is common. They're a common, generally well-tolerated replacement for CAPB, but "CAPB-free" and "glucoside-based" are not the same as "safe for you." If you have a confirmed surfactant allergy, patch-test rather than assume.
Is a sulfate-free shampoo automatically CAPB-free?
No — often the opposite. Cocamidopropyl betaine is the surfactant that most commonly replaces sulfates, so sulfate-free, "gentle" and "natural" shampoos are exactly where CAPB tends to hide. Judge a shampoo by its surfactant list, not by a sulfate-free claim on the front.
